Accessing Your Health Information
At Hatemi Hospital, you have the right to access your personal health records in accordance with applicable privacy and health information laws.
At Hatemi Hospital, all requests for access to personal health records are handled offline to ensure privacy and proper verification. To request your health information, please visit the hospital in person and speak with our medical records department. You will be guided through the process and required to complete and sign a physical request form on-site. We are committed to processing your request promptly, although it may take up to 30 days to complete, depending on the nature of the information requested.

Requests for Proof of Birth / Death / Admission
Hatemi Hospital provides official documentation related to birth, death, and admission for patients treated at our facility. For assistance, please contact the Medical Records Department directly at the hospital.
Birth Certificate
Hatemi Hospital does not issue official birth certificates. After a delivery, the hospital provides a **Statement of Live Birth**, which can be used to apply for an official birth certificate through your local municipal authority or registrar’s office.
Death Certificate
The hospital does not issue official death certificates. A **Medical Certificate of Death** is completed by the attending physician and provided to the family or funeral service provider. The official death certificate must be obtained from the local registrar or government office where the death is registered.
Admission Proof
Proof of hospital admission can be requested directly from the hospital’s Medical Records Department. Please visit in person with valid identification to complete the request process.
Corrections to Your Record
If you believe your health record contains incorrect or incomplete information, you may request a correction by visiting the Medical Records Department. Submit a written request along with supporting documentation. We aim to process such requests within 30 days.
Uses and disclosure of personal health information
Your personal health information is used and disclosed to treat and care for you and to:
- Provide patient- and family-centred care
- Support provision of continuing care following discharge or transfer to another care facility
- Conduct patient safety and quality improvement activities including satisfaction surveys
- Meet legal requirements and as permitted by law
- Fundraise for facilities, equipment and programs
- Obtain payment where necessary
